Evaluations:
Evaluations of pre-school and school-aged children take from 1 1/2 to 2 hours to administer. Evaluations for children 2 years and under take from 45 minutes to 1 hour. On rare occasions children have difficulty sitting through the evaluation. When appropriate or necessary a child may be brought back for a second observation or home/school observation at additional cost. This decision will be made between the therapist and parents, based on information gathered on the day of testing and the parent interview/questionnaire.

Evaluations include sitting for the evaluation, scoring, a written narrative report and a written treatment plan. An in-person interpretive can be arranged for an additional fee. Occupational Therapy evaluations done by other facilities will be accepted if they are current. If a treatment plan with specific long- and short- term goals was not included, this will need to be done and will be billed at the prevailing rate per hour.

Consultations:
On occasion school visits/consultations are an important part of other treatment process and are very necessary. You will be billed for these sessions at the prevailing rate per treatment hour. There may be an additional travel fee applied to these visits. These appointments are made at the discretion of the therapist, parent and teacher.

We are always available for brief phone conversations regarding your child’s progress. Phone conversations beyond 5-10 minutes will be billed as consultative services.

Cancellations:
If your child is sick it is best to cancel therapy. We follow health department regulations which state that a child must be fever free for 24 hours. In addition to this, if there is nasal discharge that is not clear or discharge from the eyes, chances are your child is contagious and should be taken to the doctor rather than therapy. If you have any doubts about whether to bring your child, call and we can make the determination together.

Billing Policy:
Clients will receive a bill suitable for insurance purposes. From time to time bills are lost and copies of bills are requested. Requests for copies of one or two bills can easily be accommodated.

There will be an office charge when copies for more than two months worth of service are requested. Payment is due at the time of service. For those wishing to pay on a monthly basis by credit card we will automatically bill your credit card at the end of the month for which services were rendered. We reserve regularly scheduled appointments for our clients. In consideration for all clients we start and end the session at the regularly scheduled time and appreciate our families’ cooperation in this policy. We believe that children who come consistently to therapy make the most progress, however, we do understand that there are times when it is necessary to cancel a session. We ask for 24 hours notice if you find you cannot make your appointment. Last minute cancellations (i.e. less than twelve hours before the designated appointment and /or no calls, no shows) will be billed for the treatment session missed at the cost to the client, and not the insurance company. Exceptions to the cancellation policy will be made for clients who are ill upon awakening. Please call and leave a message first thing in the morning (before 8 am) if your child is sick. If you wait until right before the appointment to call and cancel you will be billed. If weather is bad, we will contact you to discuss arrangements, or if you know you cannot get to your appointment, call and leave a message on the machine. We accept payments by check, VISA, or MasterCard.
